Please print out and fill the New Patient Registration Packet before coming to the office for the first time. If you do not have a printer, or do not have the time to fill them out at home, you will be asked to fill them out in the office. If you have already filled out these forms, you will likely never have to fill them out again, unless a significantly different version is produced.
